Lunenburg County Community Health Board
WHO ARE WE?
The Lunenburg County Community Health Board (LCCHB) is a group of volunteers from your local area who are working to improve health and wellness. Our role is legislated under the Health Authorities Act. We represent the people of Lunenburg County and
strive to recruit members from the far corners of our county. We encourage members to join who reflect the diversity of our county - men and women of all ages, ethnic groups, etc. We seek out those who have an interest in health and wellness and those who have knowledge of their local community.
Lunenburg County Community Health Board was the 2011 recipient of South Shore Health's Excellence in Community Health Leadership Award.

WHAT DO WE DO?
Our Community Health Board:
- Listens to the people of Lunenburg County, gathering information on health and wellness concerns;
- Develops a Community Health Plan, setting priorities for change;
- Advocates for improved health and health services;
- Takes action at the community level to put the CHB Plan into place;
- Awards Wellness Grants to local community organizations.

COMMUNITY WELLNESS GRANTS
LCCHB receives funding annually from the Nova Scotia Department of Health and Wellness to disburse to non-profit organizations that promote healthy lifestyles in our communities. Organizations can be eligible for a grant up to a maximum of $3,000.00 per year for new or existing programs that focus on improving the health of people in Lunenburg County and that support at least one of the priority areas of the Lunenburg County Community Health Plan. These priorities include: Physical Health &Wellbeing; Addictions; Mental Health & Wellbeing; Healthy Eating/Food Security; Healthy Child, Youth & Family Development.
To date, we have awarded more than $327,000 in community wellness grant funding. See a complete list of organizations that have been supported through our Community Wellness Grant Program by clicking here.

People who do not currently have a Doctor or a Nurse Practitioner OR are travelling outside of the District for care are encouraged to add their names to a database of people in need of a healthcare provider. You can do one of the following to put yourself and/or your family members in the database.

Your name(s) will be added to a database and you will be contacted when/if space becomes available.

2013-2018 COLLABORATIVE COMMUNITY HEALTH PLAN & FINDINGS REPORT FROM NOV 2011 COMMUNITY CONSULTATIONS
Since 2010 the Lunenburg County Community Health Board has been working collaboratively with the Queens Community Health Board to develop a five year Collaborative Community Health Plan for the district (Lunenburg & Queens counties) for 2013-2018. Focus groups were held in communities across the district in November 2011 to gather feedback from community members. This feedback, along with a Community Profile, were the main tools used by the two CHBs at a workshop held in April 2012 where they identified the priority area themes for the upcoming plan. At a workshop in June 2012, the two CHBs came together again to determine the recommendations that would be made to South Shore Health in the 2013-2018 Collaborative Community Health Plan. The 2013-2018 Collaborative Community Health Plan was presented to the South Shore Health Board of Directors on September 20, 2012. Under the Health Authorities Act, the district health authority is required to consider the community health plans developed by Community Health Boards in their annual business planning and report back to the Community Health Boards annually on the work they are doing.
