The Diabetes Education Centres (DEC) are part of Chronic Disease Management Program at South Shore Health.

The Centres provide information on how to care for diabetes.  If you have diabetes (type 1 or 2 – adult , child, adolescent, or young adult), are pregnant and have diabetes, or have been told you have a mild form of diabetes or prediabetes, the centre is here to help you.

The centre promotes self-management to improve quality of life and to reduce the risk of developing diabetes complications. 

Our program provides a number of ways to learn about your diabetes:

  • Individual instruction. (Individual appointments with the nurse and dietitian.)
  • Group instruction. (This provides you with an opportunity to learn from and with others who also have diabetes or prediabetes.)
  • Insulin Start. (If you need insulin, this means that you can start taking your insulin as an outpatient.  You may come to the centre to learn how to use insulin rather than being put in the hospital.)
  • Survival skills. (This short, 3-hour session provides answers to commonly-asked questions about your diabetes. This course is offered 2-3 times per month on demand.)

Fishermen's Memorial Hospital, Lunenburg
3rd floor near Ambulatory Care
Monday to Friday from 8 am - 4 pm 
(Satellites to: New Germany Health Center 2nd and 3rd Thursday of each month; Chester Health Center 2nd and 4th Wednesday of each month).

Queens General Hospital, Liverpool
First Floor, near the Lab
Wednesday and Thursday from 8 am - 4 pm
(Satellite service to the North Queens Medical Centre every 3rd Thursday of the month.

South SHore Regional Hospital, Bridgewater
Main Floor Chronic Disease Care, 2nd and 4th Monday of each month 8 am to 4 pm

How can I make an appointment?

Your first appointment will be made after your doctor or nurse practitioner has referred you to our centre.  The staff of the diabetes program will arrange follow-up appointments. If you are interested in the education programs, you can call without a doctor referral.
